They get talked about as if they are two versions of the same thing. They are not. Dermal fillers and anti-wrinkle injections solve two completely different problems, and the wrinkle you are looking at in the mirror usually tells you which one you need.

The One Distinction That Explains Everything
Almost all the confusion clears up once you understand dynamic versus static wrinkles.
Dynamic wrinkles appear when you move your face. Frown lines, forehead lines and crow’s feet around the eyes all show up when you express yourself, and soften when your face is relaxed. They are caused by muscles contracting.
Static wrinkles are the ones visible even when your face is completely still. Deeper folds around the mouth, lines from the nose to the corners of the lips, hollowed cheeks and thinning lips. These are caused by lost volume and collagen rather than movement.
Anti-wrinkle injections treat the first kind. Dermal fillers treat the second. Almost everything else follows from that.
How Anti-Wrinkle Injections Work
Anti-wrinkle injections use a purified botulinum toxin, injected in tiny amounts into specific muscles. It gently relaxes those muscles, so they stop pulling the skin into a crease every time you make an expression. The line softens, and because the muscle is no longer creasing the skin, deeper wrinkles are discouraged from forming over time.
They work best in the upper face, where movement drives the lines: forehead, between the brows, and around the eyes. Only the muscles treated are affected, so the rest of your face moves normally, which is the key to a natural result rather than a frozen one.
Results are not instant. The muscle takes time to respond, so you usually notice softening within three to five days, with the full effect around two weeks. It typically lasts three to four months before a top-up is needed.
How Dermal Fillers Work
Dermal fillers do the opposite job. Rather than relaxing anything, they add volume and structure beneath the skin.
Most modern fillers are made from hyaluronic acid, a substance your skin produces naturally to stay hydrated and supported. Injected carefully, it fills a static line, restores volume to areas that have hollowed with age, and can enhance features like the cheeks, lips and jawline.
Because volume is being placed directly, results are visible almost straight away, though there can be some initial swelling that settles over a few days, so the final look is best judged after that. Fillers also tend to last longer than anti-wrinkle injections, often somewhere from six to eighteen months depending on the product and the area treated.
One reassuring feature of hyaluronic acid fillers is that they are partly reversible. If needed, an enzyme can be used to dissolve them, which is not possible with muscle-relaxing injections.
So Which One Do You Need?
Here is the simple way to think about it. Look in the mirror with your face relaxed.
If the line you dislike disappears when your face is still and only shows when you frown, smile or raise your brows, that is a dynamic wrinkle, and anti-wrinkle injections are usually the answer.
If the line or hollow is still there when your face is completely relaxed, that is a static concern from volume loss, and dermal filler is more likely to help. Thin lips, flat cheeks, and the deeper folds around the mouth fall into this group.
The honest reality is that many people have both, and the two treatments work beautifully together. Relaxing the muscles that cause movement lines while restoring the volume that has been lost addresses the whole picture rather than half of it. It is very common to have both at the same appointment, tailored to what your face actually needs.
